§ What a permit has to contain
- Description of the circuit and equipment, and its location.
- Description of the work to be performed.
- Justification for why the work must be performed energized.
- Description of the safe work practices to be employed.
- Results of the shock risk assessment — voltage, limited and restricted approach boundaries, shock PPE.
- Results of the arc flash risk assessment — incident energy or PPE category, arc flash boundary, arc flash PPE.
- Means employed to restrict access by unqualified persons.
- Evidence of a completed job briefing, including job-specific hazards.
- Energized work approval signatures.
§ Two things the 2027 edition changed here
The permit moved. It was 130.2(B); it is now its own section at 130.3. The requirements did not change technically, but every form and procedure citing the old number is now wrong.
The additional person. Where the permit specifies shock or arc flash PPE, a second person trained in emergency response under 110.4(C)(1) must be present, outside the greater of the two boundaries. Add the field to your form — it is on this one.
Blanket permits covering "electrical maintenance in the switchgear room" for a quarter. A permit is per-task, and the justification has to be specific to that task. A standing permit is a filing habit, not a control.
